Things to Bring to Your Appointment
If you are not able to obtain paper copies of these records please email them all together to vbtrustee@vanburentownship.org
Not everything on this list will apply to you. The most important items are bolded. If you are missing these items without good reason this may cause your application to be denied. Please be prepared to explain why any of these items that pertain to you are missing.
The Van Buren Township Trustee program must document all expenses and income according to the Township Assistance Guidelines and Eligibility Standards. The following documents as well as a 100% completed application must be provided at the time of the Township Assistance interview or application cannot be accepted.
Identification for all household members (Must show current V.B. Twp. Address)
Social Security cards for all household members
Pay check stubs for the last 30 days (all adults) or proof of income including paycards
Social Security, VA or Welfare Award Letters
Tax return statements showing any refunds if that refund was received within 6 months of your appointment.
Proof of expenses paid in past 30 days - either an itemized bank statement or receipts for all purchases if you do not have a checking account.
Current gas, water & electric bills
Monthly rental agreements, mortgage statement, or lease
Food Stamp Letter showing amount recd. And Case # (Print from Website)
Shelter Verification - Must be filled out by Landlord within one week of your appointment. This form is available on the Township Assistance page of our website.
Section 8 Award Letter
Proof of job search for each unemployed, non-disabled adult in household (minimum of five completed job applications completed within 30 days of application.)
Proof of pending Social Security Disability application
Proof of out of pocket medical bills paid, prescription co-pays
Car insurance statement
Furniture rental contracts/payments
Annuities/Pension Statements
Proof of Court Costs
Proof of money borrowed or received as a gift
Registration or Titles to all motor vehicles owned, leased or borrowed by any household members
Child Support Decree/Print out from prosecutor
Any stocks and bonds owned by household members
Deeds/Titles to all properties owned
Receipts for income from selling or pawning personal items
Home or car repair receipts
Proof of workmans compensation benefits
Income from rented or shared housing
Money received from selling Blood/Plasma
Lawsuit, Disability and /or insurance Settlements
Phone or Cell phone bills contract or prepaid
Information on Trust Funds
SCCAP Utility Assist Award Letter
Cable or Internet services statement
Medical restriction letters if client or other adult in household has any medical work restrictions
Proof of veteran status
